If your PolyCard (Campus ID) is lost or stolen, report it immediately to prevent anyone else from using your membership. Simply call Customer Service at (805) 756-5939. You may also log into the MyCalPoly portal (http://my.calpoly.edu), go to the Money Matters tab, click on the Campus Express Club channel, then "Lost/Stolen Card" on the right. Then go to Customer Service in the atrium of building #19 for a replacement.
