Campus Express Club Terms and Conditions
PLEASE READ THESE PROVISIONS
Campus Express Club ("Campus Express") is offered by Campus Dining, a unit of the Cal Poly Corporation, to Member [student, staff, faculty]. By using your Campus Express Club ("Campus Express") membership, the Member agrees to the following terms and conditions.

USING CAMPUS EXPRESS. Campus Express is used to purchase non-food as well as food items from venues and services around campus, and is a dollar for dollar declining balance account. Value rolls over from quarter to quarter and from academic year to academic year as long as the account stays active.

MEMBERSHIP. All students, faculty and staff are automatically enrolled in the Campus Express Club when they are issued a PolyCard, and become Members when they add value to the PolyCard for Campus Express, per the terms of this agreement.

PLAN ACCESS. Campus Express Club is a type of "Value Account Plan" that is accessed through the magnetic strip on the back of the Student's University ID card ("PolyCard"). Students must present the PolyCard at the cash registers to purchase goods with the Dining Plan. It is the Student's responsibility to acquire the PolyCard before using the Dining Plan. Campus Dining provides all food service support at the University, and Campus Dining Customer Service ("Customer Service") is located in the Atrium, Building 19, 805-756-5939, in the same office as the University's PolyCard office.

VENUES. Other venues may be added or changed (or names changed) throughout the year. Venues may be open for only some Meal Periods, which will be posted at each venue and at this website. www.calpolycorporation.org/campusdining/hours.asp
Campus Express can be used at El Corral Bookstore, Pony Prints, campus computer labs, Health Services and for equipment rental at the Recreation Center, and for food services at all restaurants on campus, which include Tacos To-go, Dexter Subs & More, Lucy's Juice, The Avenue, 19 Metro Station, BackStage Pizza, Julian's Patisserie, VG Cafe, The Sandwich Factory, Sage Restaurant, Starbucks, and the Poly Canyon Village restaurants (Peet's Coffee & Tea, Einstein Bros. Bagel, and Jamba Juice), the convenience stores (Campus Market and Village Market), and some vending locations, including the Park.

Occasionally, offers may be advertised for Bonus value to be added to the account if Member adds a minimum value during a certain period.

Value may be added by logging into the MyCalPoly portal (http://my.calpoly.edu), go to the Money Matters tab, click on the Campus Express club channel, by credit card Visa, MasterCard, or Discover or by telephone to Campus Dining Customer Service, the Cal Poly Corporation Cashiers office in Building 15, and EXPRESS STATIONS located on campus, by mail to Campus Dining Customer Service, San Luis Obispo, CA 93407. You can also add value using by telephoning Customer Service at (805) 756-5939 or Cal Poly Corporation Cashier at (805) 756-2849.

Online value additions by credit card are usually instantaneous but may be delayed due to network traffic. Restitution is required for overdrafts resulting from purchases made while the cash register system may be off-line, with the computer server showing updated balances.
CANCELLATION. A Member can request cancellation of the Campus Express account at any time by completing the cancellation and Refund request form at Customer Service. Campus Dining will cancel a Campus Express account when it has been inactive for about five months.

REFUNDS. Refunds may be requested at any time from Campus Dining Customer Service, and Members should request a Refund of unused value prior to ceasing use of the account or departing the University.
If the account was paid by credit card, the funds will be refunded to the credit card account of the person who into the account. If the account was paid by cash or check, the funds will be refunded by check to the Student. Refunds are subject to a $15 processing charge.

Partial Refunds are not given, and any Campus Express bonuses are not refundable.

It is the sole responsibility of the Member to contact Customer Service to request a Refund.
FORFEITURE OF VALUE. If a Refund of unused Value is not requested by the Member prior to a Campus Express account is closed by Campus Dining or the Member, the remaining Value will be forfeited. No bonus dollars will be refunded when an account is closed by Campus Dining or by the Member.

PURCHASING WHEN STUDENT MAY HAVE MULTIPLE VALUE ACCOUNT PLANS. There are multiple "Value Account Plans" available to Students on campus and all are accessed through the PolyCard. The cash registers are generally set to deduct value from the PolyCard in the following order: Dining Credits, Plu$ Dollars, PCV Dining Dollars, and then Campus Express Club. The Student can ask the cashier in advance to deduct a purchase from a specific Value Account Plan for a particular purchase, if that Value Account Plan is valid for that purchase.

NO TRANSFERS. The use of Campus Express is limited to the Student whose name appears on the PolyCard. The value and the Plan may not be resold, assigned, transferred, or used by anyone else without prior authorization of Campus Dining.

UNAUTHORIZED USE. Unauthorized use of the PolyCard or the Campus Express Club value, including use by a party other than the person identified on the PolyCard, or tampering with or altering the PolyCard may warrant confiscation and possible disciplinary action by the University, a fine, and possible forfeiture of value expended.

LOST OR STOLEN CARDS. The Student is responsible for any charges on a lost or stolen PolyCard. Student is responsible for maintaining the integrity of the PolyCard in good working order. Magnetic fields, such as those created by TV, stereo, microwave, or magnetic clasps of wallets and purses can make a card unreadable, as can heat, water and eel skin. A minimum charge of $5 may be charged by University's PolyCard office to replace a PolyCard for any reason. The PolyCard must be readable by Campus Dining's cash registers in order for a purchase to be transacted. It is the Student's responsibility to report a lost or stolen PolyCard by:

Logging into the MyCalPoly portal (http://my.calpoly.edu), going to the Money Matters tab, clicking on the Campus Express Club channel, then clicking "Lost/Stolen Card" on the right, which instantly deactivates the PolyCard and all value account plans on the PolyCard;

Visiting the Customer Service office; or

Calling Customer Service at 805-756-5939 during weekday business hours.
NO INTEREST. No interest or other earnings will be credited to the account by Cal Poly Corporation.

MISCELLANEOUS. Any situation not specifically addressed herein will be resolved at the sole discretion of Campus Dining. Campus Dining cannot be responsible for payments that are lost, late, misdirected, mutilated, or delayed. Campus Dining is not responsible for incomplete, incorrect, or illegible entries made by the Member during the application, Refund, or add value process.

AMENDMENT OF TERMS. Campus Dining may amend the terms of this Contract at any time.

FACULTY AND STAFF MEMBER DISCOUNT AT SELECTED VENUES FOR LIMITED PERIOD. Campus Dining will provide a 15% discount to Faculty and Staff Members using Campus Express on food and beverage purchases from September 14, 2009, through June 30, 2010 at the following venues: The Avenue, BackStage Pizza, Dexter Subs & More, Julian's Patisserie, Lucy's Juice, 19 Metro station, Sage Restaurant, Sandwich Factory, VG Café, Campus Market and Village Market. This discount is not provided on any purchases from Campus Catering, Starbucks Coffee, Peet's Coffee & Tea, Einstein Bros. Bagel, Jamba Juice, the Park, any vending machines, or any other facility or service that accepts Campus Express for purchases.
